When you register with MyIR, you can also download, print and store a Mississippi Certificate of COVID-19 Vaccination showing the current vaccination status for you or your family members. The MSDH Immunizations Office can help with updating or correcting your records. Once you have located your account, check the Documents section for an immunization history or Form 121 to download or print.

If your information does not exactly match an official immunization record, use the green chat bubble on the bottom right of the screen to provide extra details to help locate your records.

If your information matches an entry in the immunization registry, choose Take Me to My Account to see immunization records, immunization needs, and to download a Form 121.

Fill out the required personal information to help the system locate immunization records for you and your family.

Once you have created your account, you can use it to look up records at any time.

Choose Sign Up, then verify your account by phone or e-mail.

Enter your name, e-mail address, and create a password.

If you are using MyIR for the first time, create an account. Print or save Mississippi COVID-19 Certificates of Vaccination for yourself or family members.If your child is up to date with age-appropriate vaccinations, print out a certified copy of the Form 121 required for school registration.
